Greetings Commissioned Corps Officers,
As summer approaches, I’m sure many of you are planning or about to take a vacation. If your trip involves leaving the country, this is a friendly reminder:
Effective as of June 12, 2017, Director of National Intelligence (ODNI), Security Executive Agent Directive (SEAD) 3 requires all HHS/IHS staff who are (1) eligible for sensitive positions, (2) have access to any type of classified information and/or (3) maintain a National Security clearance to report a variety of life events, including all non-work-related foreign travel and substantive foreign contacts to their local security office, Office of National Security (ONS).
SEAD 3 requires enhanced additional reporting of foreign travel, foreign contacts, and conduct/behavior. The revised reporting requirements are placed to protect the United States from National Security breaches and also to prevent significant harm to the interests of the United States.
As an HHS affiliate with National Security background credentials, you are required to report foreign travel and activities to HHS ONS by way of forms found below. As a holder, you are required to utilize the following forms for reporting actions:
- HHS-844: Pre-Travel Approval Form: For Pre-OCONUS Travel (To be used 30 days prior to personal travel outside of the United States)
- HHS-845: Post-Travel Follow-up Form: For Post-OCONUS Travel (To be used within 5 days of return from travel outside of the United States)
- HHS-846: General HHS Travel Form: To Report Actions and/or Activities (To be used for any required reporting actions related to one self and/or peers.
Upon completion of any of the attached forms, please submit them only to International@hhs.gov and PersonnelSecurity@hhs.gov for processing and approval.
